Supporting people with learning disabilities 14+years


Annual health checks are an important opportunity to identify unmet need, improve outcomes, and support earlier intervention. I work with practices to strengthen how these checks are promoted, prepared for, delivered, and followed up.

Support can include:

  • improving uptake and engagement 
  • reviewing invitation and recall processes 
  • considering accessibility and communication 
  • making health checks more person centred 
  • helping teams understand what good practice looks like 
  • identifying missed opportunities and gaps in follow up 


Supporting people with severe mental illness 18+years

Health checks for people with severe mental illness should not be seen as a routine exercise. They are a valuable opportunity to reduce inequality, improve physical health oversight, and support more joined up care.

Support can include:

  • strengthening practice systems and pathways 
  • improving consistency and quality 
  • supporting staff confidence and understanding 
  • identifying where patients may be missed or not meaningfully engaged 
  • improving follow up and continuity 


Supporting people with dementia

People living with dementia may face barriers to accessing and benefiting from primary care reviews. I support practices to consider how dementia informed, person centred approaches can improve the experience and value of care.

Support can include:

  • improving access and communication 
  • supporting carers and families within the process 
  • reviewing the quality and purpose of dementia reviews 
  • considering reasonable and proactive adjustments 
  • helping staff recognise what matters most to patients and carers
